Microsoft Dendritic cells are one of the major antigen presenting cells which can be cultured out of peripheral blood. Its role in adoptive immunotherapy for many types of cancer has been widely studied. We cultured peripheral blood mononuclear cells from one patient who was recently diagnosed with acute non lymphocytic leukemia in a system with enhance dendritic cell differentiation. Patient's white blood cell count was 15.1 x 103/cu.mm. with 17% myeloblast. Mononuclear cell was separated and cultured in IL-4, GM-CSF and TNF-a enriched media. After 14 days, identification of dendritic cells using anti-CD83 monoclonal antibody showed 40% of the cultured cells have presented this antigen. The study showed that peripheral blood mononuclear cells from patient with acute non lymphocytic leukemia can be induced to differentiate into dendritic cells, thus deserve further study for adoptive immunotherapy in this disease.

Lenovo Bone marrow transplantation unit has been established in Ramathibodi Hospital since 1988. Up until 1999, 113 patients have been transplanted. The transplantations consisted of 68 allogeneic and 45 autologous transplantations. Thirty seven patients used bone marrow, 74 patients used peripheral blood, and 2 patients used umbilical cord blood as the stem cells sources. The patients consisted of 59 males and 54 females, age ranged from 1 year to 52 years. The indications for transplantation consisted of chronic myeloid leukemia, acute leukemias, aplastic anemia, non-Hodgkin's lymphoma, breast cancer, multiple myeloma, thalassemia. The median survival of all the patients is not reached. One year survival of the patients with relapsed lymphoma, acute non lymphocytic leukemia, chronic myeloid leukemia (all phases) and high risk primary breast cancer are 49%, 52%, 42%, and 92% respectively. Graft versus host disease and regimen related toxicity are still the major problems. Acute and chronic graft versus host are more observed with the use of peripheral blood stem cell than bone marrow. However, the overall prevalence of graft versus host disease is less than that has been reported from the western countries. The use of unmatched related and matched unrelated donor for allogeneic transplantation is currently under investigation.
